Team Livingston: Jazmin Arteaga

Jazmin Arteaga’s exceptional work managing data for the combined Telemedicine, Alliance, and CATCH program patients deserves recognition.

Jazmin’s career with Livingston dates back to December 2010. She has worked at Livingston Caregivers as a personal caregiver to patients and provided clerical assistance to the Hospice program. She moved to support the population management team — Telemedicine, Alliance, and CATCH — in December 2015.

The success of patient care in population management is dependent upon accurate data tracking and patient coordination. The position requires organization, efficiency, attention to detail, creativity and the ability to multi-task — all qualities Jazmin possesses. She is also extremely professional and pleasant when coordinating assessment visits for patients with nurses.

But beyond office duties, Jazmin possess a spirit of love and kindness. “She doesn’t just do her work; she makes people’s lives better” is they way Jazmin is described by her peers.

Livingston is a non-profit health care provider with a mission to care for everyone in our community. Jazmin is a shining example of how to put the mission into action. She is personally committed to helping homeless patients in the CATCH program by assembling individual bags of food, water, toiletries, tissues and wipes to be taken out by assessment nurses.
