Levels of Care

Personal Care
Personal care includes general assistance and oversight for all activities of daily living, such as bathing, dressing and grooming. This may also include general household help such as laundry, linen changes, light housekeeping, meal planning and preparation. There may be a need for ambulation, transfers and mobility assistance and/or transportation to appointments, shopping and other errands. Medication reminders, companionship and whatever personal care is needed can be provided. Along with our personal care, our Care Givers also promote the interests and activities of our clients to stimulate and involve them in their activities of daily living. After an initial assessment by one of our skilled professionals a customized care plan is devised.
Respite Care
The focus of Respite Care is to offer the family peace of mind and a needed break from the responsibilities of nurturing and caring for a family member. Respite care provides much needed assistance to the caregiver by providing attention to their loved one. This may be due to vacation plans, recuperation from an illness, or the need to have a break from the daily stress of caring taking. There may be times when just a few hours are needed or 24 hour care may be required for several days. All of the services of Personal Care are included as needed.
Transitional Care
Transitional care is usually short term assistance to assist someone who may be recovering from surgery or an illness and needs help in the home. The goal is to support and assist the client to be as independent as possible so they can return to their normal activities of daily living. All of the services offered in our Personal Care package can be provided as needed.
