Homebound Senior Care
Livingston has joined in collaboration with Caregivers: Volunteers Assisting the Elderly and Our Community House of Hope to promote health, well-being, dignity, and independence of frail, homebound seniors. This collaboration is being generously funded by Dignity Health.
Our program introduces a coordinated approach to reduce the risk for low-income seniors by providing layers of support as they age and their needs change with a focus on quality of life through the end of life. Through this continuum of support we deliver the message that this is not a journey that the senior takes alone. The beauty of this program is that it provides not only psychological and social support through engagement; it also incorporates medical support through active volunteers and caregivers monitoring the condition of each senior participating in our continuum. The challenge is to ensure that seniors throughout Ventura County are recognized and supported through programs that are sensitive to the unique aspects of aging. The three collaborative organizations, with the support of Dignity Health, are focused on placing seniors at the center of care, through a nuanced program to address individual issues.
